Chronic diseases—particularly type 2 diabetes, hypertension, and cardiovascular disease—remain leading causes of illness and death globally. In the United States and similar health systems, these conditions do not affect all populations equally. Minority communities, including Black, Hispanic/Latino, and some Indigenous populations, experience higher prevalence, earlier onset, and more severe complications.

From a nutrition and health coaching perspective, this pattern is not explained by individual behavior alone.
